Simply call (404) 219-7500 mobile or (404) 816-6262 office. I am often not immediately available to answer but you may leave a message on my confidential voicemail. I make every effort to return your call within the same day it was placed with the exception of weekends or holidays.

The initial paperwork, which will need to be completed prior to your first appointment, can be accessed from the link below. You may bring the completed forms with you to your first appointment, or fax them to (404) 816-6264.

$150.00 for an initial Diagnostic Assessment
$125.00 per 50 minute Individual Psychotherapy session
$125.00 per 50 minute Couples, Family Psychotherapy session
$65.00 per 50 minute Group Therapy session
 

I keep some session times reserved for sliding scale fees.  While these times are often full, please feel free to inquire if you are in need of a sliding scale fee.
 

Billing and Payments:

I accept cash, checks or credit cards. Payment is due at the time of service. I do not accept payment directly from insurance companies.  However, I can provide you with a receipt for services that includes all of the information you will need to file a claim with insurance company. If you are eligible for out of network benefits, you may be reimbursed based on your insurance plan.

Visa, MasterCard and American Express are all accepted.

Cancellations:

It is my policy to not double book appointments. Each appointment is a block of my time reserved exclusively for a single client. When a client misses an appointment or fails to provide 24 hours notice for cancellation, the client has occupied the reserved block of time regardless of any service rendered. Therefore, the client will be charged in full for any appointments missed or cancelled without 24 hours notice.
